If you are a current or former customer of A.J. Blosenski, Inc. (AJB), you may be entitled to benefits from a class action settlement.

All Claim Forms must be submitted online no later than December 2, 2024, or, if mailed, postmarked or received no later than December 2, 2024.


How to File Online:

After clicking the button below, you will be asked to provide the Unique ID you received on the postcard or in the email you may have received from the Settlement Administrator. If you did not receive a postcard or email or have lost yours, please contact the Settlement Administrator to obtain your Unique ID.

After submitting your completed Claim Form online, you will receive an email with a confirmation code for your completed submission. Receipt of a confirmation code means your Claim Form was successfully submitted. If additional information is required to complete your Claim, you will be contacted by the Settlement Administrator.

How to File by Mail or Email:

If you wish to submit a Claim Form via standard mail, you may download a copy of the Claim Form here. You will need to provide all of the information requested on the Claim Form, sign it, date it, and mail or email it to the Settlement Administrator using the contact information below.
